A Telecom Pensioner can avail the opportunity of drawing his pension either from a Public Sector Bank or from the Post Office. They also have the option to switch over at any time from the Bank to the Post Office or vice-versa as per their convenience. Check-List for Pension Cases

1.

For Retirement Cases.

Form 3: Showing details of Family.

Form 5: Application for Pension.

Form 7: Form for Assessing Pension/Family Pension and Gratuity.

Service-Book duly completed in all respects including specific entries regarding suspension, Up gradation, promotions etc as on date of submission showing inter-alia up to date certificates of Service verification and Remission of pension Contribution.

Last Pay certificate showing paid ending date of Superannuation with NIL outstanding.

Un conditional Vigilance Clearance certificate ending date of Retirement issued by the competent authority.

A Copy of retirement order.

A set of specimen signatures (Three copies) duly attested by a Gazetted Officer or Three slips bearing the left hand thumb and finger impression duly attested by a Gazetted Officer in case the pensioner is not literate.

Three copies of passport size Joint photograph with spouse duly attested by Head of the Office.

Three slips showing the particulars of height and identification marks duly attested by a Gazetted Officer.

The above documents /papers are required to be forwarded to CCA office through a forwarding letter in form 8 which shall serve the purpose of No demand certificate also

In case of revision in pay causing revision in pension, gratuity, commutation, etc.; revised Fixation of Pay along with calculation sheet of pension, revised LPC etc. should be submitted.

In Cases of Death.

In case of death of a government servant, following should be submitted to Communication Accounts Office through A.O. (TA) of the S.S.A concerned. All documents are to be countersigned by Head of the Office or any other officer so entrusted by head of the office.

Form 14: Application for grant of Family Pension.
Form 12: Application for grant of Death Gratuity.
Service-Book duly completed in all respects including specific entries regarding suspension, Up gradation, promotions etc as on date of death showing inter-alia up to date certificates of Service verification and Remission of pension Contribution.
Last pay Certificate with Nil outstanding ending date of death.
A copy of death certificate issued by the competent authority.
Three specimen signatures or left hand thumb and finger impression of the claimant or guardian (in case of minor) duly attested.
Three specimen signatures or left hand thumb and finger impression of the claimant or guardian (in case of minor) duly attested.
Three copies of passport size photographs of the claimant or guardian duly attested.
Three copies of descriptive roll of the claimant or guardian duly attested indicating height and personal marks.
Postal Address of the claimant.
The above documents /papers are required to be forwarded to CCA office through a forwarding letter in form 19.
